Haven't seen your rig yet, so I don't know how many points your character's mouth has, but try this: Move different points for the different smart bones. That is, try to use only the points at the edges for the happy/sad expression smart bone, and the points at the middle for open/close mouth smart ...

It might help if your gradient control was centered in the circle. Also it looks like you used "smooth" interpolation on the keys for the rotation. Smooth interpolation starts and ends slow, and speeds up in the middle. Perhaps change the interpolation on those keys from smooth to linear.

I remember one of the animators around here had mentioned problems with getting gears like that to mesh cleanly, but you seem to have got it down pat.

Without seeing the file itself, I can only guess. It sounds like a binding problem. Make sure all your points are bound to the correct bones. Without seeing the animation, I can only give some guesses. It sounds like you have the hand moving in relation to more than one bone at once. Check to make sure that your hand is only bound to the appropriate bone, and check the bone strengths of the nearby bones to make sure there is only one bone...

"Squint" lines could also be added to the character at frame 0, but with the line width set to zero, so that the lines would be invisible, Then the line width could be animated at whatever point you require. Also you could set up a "squint" smart bone that would lower the eyelids...
